Inquiries regarding personal information (request for disclosure, etc.)

C&G Systems Inc. (hereinafter referred to as the “Company”) will accept requests from persons who are information subjects or their agents related to disclosure, etc. (disclosure, correction, addition, deletion, suspension of use, erasure, provision to third parties) of personal information retained by the Company. In order to ensure identification of applicants, we only accept written requests. When making a request, kindly print out the form below, fill in the required items, enclose the documents for identification of the person and handling fees specified below, and mail them to the inquiry counter.

(Requests from the subject of the information)

1. Request form (with the person’s officially-registered seal affixed)
2. Original copy of seal impression certificate (within 3 months of issuance date)
3. Document for verification that the applicant is the subject of the information (either (1) or (2) below is required)
(1)Copy of driver’s license or passport: either
(2)Copy of health insurance certificate or pension book, and copy of resident card: one of each
4. Postage stamps of ¥1,000 face value

(Requests from agents)

In the case of a requests from an agent, in addition to the documents in 1-4 above, the following documents as well must be submitted:

1. In the case of an authorized agent

Documents to confirm the fact that the applicant is authorized by the subject person of the information (both (1) and (2) below are required)

(1)Letter of proxy (with the officially-registered seal of the subject person affixed): 1
(2)Original copy of seal impression certificate of the subject person : 1

2. In the case of a legal representative

3. Postage stamps of ¥1,000 face value

When we cannot respond to requests

When we cannot respond to a request due to provisions of the laws and regulations, or when we have not retained the personal information requested, we will mail a response to that effect (kindly understand that we will not refund the fees).

In the case of incomplete documents

If the submitted documents are incomplete, the Company will contact the applicant from the inquiry counter. However, if the applicant fails to respond with corrections or addition of documents, etc. within one month, the Company shall deem the request not to have been made.

Handling of submitted personal information

Personal information submitted upon request will be used only to the extent necessary for work related to the request. The submitted documents will not be returned.
